We’re building our team in **Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care.** ## The mission of this role **Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.** ## Objectives 1. **Carry a full, high-quality caseload:** ramp to **~28 productivity points/week** at target visit quality. 2. **Own your patients' outcomes:** drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. **Be a referral-source ambassador:** every discharge planner, ALF, and physician you touch should *want* to send the next patient to us because you were responsive and excellent. 4. **Set the standard others copy:** your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. ## What you'll own * **Own the caseload as a case manager:** you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits. * Complete comprehensive in-home assessments and deliver skilled PT visits. * Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. * Set measurable goals, track objective progress, and adjust treatment quickly when patients stall. * Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. * Manage treatment and outcomes directly, including **PTA supervision** where appropriate. * Coach patients and caregivers with practical home programs that actually get done. * Document promptly and thoroughly — defensible, efficient, and consistent. ## **Who you are / how you operate** * **Dedicated clinician:** home health experience strongly preferred * **High ownership:** you treat your caseload like your own panel and your pod like your own business. * **Open mindset:** Energized by the opportunity to shape the playbook and refine SOPs * **Tech-forward:** You want to leverage software to accelerate the charting process, and you'll give us blunt feedback to make it better * **Trust-builder:** Patients, families, and referral partners believe you because you do what you say. ## **Must-haves** * **Active Michigan PT license (in good standing)** * Strong clinical judgment and comfort working independently in the field * Comfortable escalating changes in condition to RN/MD and documenting rationale clearly * Strong communication and documentation * Reliable follow-through on orders, frequencies, and patient goals * Current driver’s license and comfort with daily travel across assigned territory * Able to carry/transport basic therapy equipment as needed * Competency in core home health PT scope + Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations + Strengthening, functional mobility, and home safety assessment/modification education + Patient/caregiver coaching with practical home programs ## **Nice-to-haves** * Prior **home health** experience * Deeper clinical experience in high-signal PT populations * Experience supervising/mentoring **PTAs** and collaborating on care plans * Strong organization and time management ## Our guarantee * **Full-time target: ~28 productivity points/week**; clinicians earn **more** for exceeding it. ## The rest of the package * **$1k bonus for referrals** (we’re growing)! * **Benefits** for full-time (medical/dental/vision, PTO) ## Apply [Apply on Fira Health]() Canonical job page:
