The Admissions Patient Coordinator serves as a key patient-facing role within the Admissions Department, supporting patients throughout the Initial Home Health Care admissions and authorization process. This position is responsible for coordinating physician appointments, nursing assessments, home visits, caregiver onboarding, and patient communication to ensure timely and effective initiation of services. The Admissions Patient Coordinator acts as the primary point of contact for patients and families, providing education, support, advocacy, and ongoing communication throughout the process. Responsibilities include attending physician appointments, coordinating documentation and scheduling, facilitating communication between departments, assisting with onboarding and workflow progression, and ensuring patients receive a positive and supportive experience from initial approval through care initiation. This role requires strong organization, communication, problem-solving, and time management skills, as well as the ability to work effectively in a fast-paced healthcare environment while maintaining a patient-centered approach to care. Candidates should be compassionate, adaptable, detail-oriented, and comfortable collaborating with patients, caregivers, healthcare providers, and interdisciplinary teams.
