Develop and execute outreach and community engagement initiatives to support enrollment growth for Seen Health’s PACE program. Build and maintain relationships with physicians, hospital discharge planners, post-acute facilities, community organizations, and other referral sources to generate qualified participant referrals and achieve enrollment targets. Coordinate and implement public relations activities, including execution of annual plans to meet enrollment objectives. Provide education to referral partners and community stakeholders regarding Seen Health’s services. Track outreach activities and enrollment metrics, prepare reports on referral trends and goal attainment, identify process improvements, and ensure compliance with applicable regulations and internal policies. Regular travel is inherent to the position and required to physicians’ offices, hospitals, community organizations, outreach events, and other settings throughout the San Gabriel Valley community to conduct in-person outreach activities, maintain referral relationships, represent Seen Health within the community, and communicate directly with external stakeholders and prospective participants. Fluency in Mandarin or Cantonese required due to the significant Mandarin- and Cantonese-speaking participant population served by Seen Health’s PACE program and the need to communicate effectively with community members, caregivers, referral sources, and community organizations within the local service area.
Requirements: Employer will accept master’s degree, or foreign equivalent, in Communications, Healthcare Administration, Gerontology or other related field. In lieu of a master’s degree, employer will accept a bachelor’s degree, or foreign equivalent, in Communications, Healthcare Administration, Gerontology or other related field, and 2 years of experience in the job offered or an outreach specialist-related position.
Any amount of experience in the additional requirements below will be acceptable unless a specific amount is noted. Prior academic background or work experience to include the following skills:
- Developing and implementing outreach plans to achieve defined referral or enrollment goals;
- Coordinating and executing outreach and community engagement initiatives to support growth objectives;
- Building and maintaining professional relationships with healthcare providers, community organizations, referral partners, or other stakeholders;
- Knowledge of healthcare outreach regulations and compliance requirements, including familiarity with DHCS and CMS guidelines applicable to PACE programs;
- Tracking outreach activities, monitoring referral trends, and preparing reports regarding enrollment performance or goal attainment;
- Conducting community-based outreach or engagement efforts tailored to senior, underserved, or culturally diverse populations.
- Fluency in Mandarin or Cantonese required.
Salary: $72,093 to $93,000 per year
